Our Services

Specialist & Forensic Support Coordination (Level 2 & 3)

We provide expert support coordination for participants with complex psychosocial needs, forensic involvement, or multiple service systems in place. Our team works closely with NDIS, clinical, justice, and community services to ensure supports are effective, coordinated, and aligned with each participant’s goals

Reintegration & Mentoring Support

We specialise in assisting participants transitioning from custody to community. Our Reintegration Caseworker and Specialist Coordinators collaborate with correctional facilities, forensic mental health teams, housing providers, and support services to ensure safe, stable, and seamless reintegration.

Crisis Response & Risk Management

Our team is experienced in managing high-risk and crisis situations. We provide responsive crisis support, safety planning, and risk management strategies to protect both participants and the community.

Collaborative Case Management

We coordinate with psychologists, psychiatrists, allied health professionals, social workers, housing services, and justice teams to develop unified, outcome-focused support plans that promote stability, safety, and meaningful progress.

Advocacy & Systems Navigation

We advocate strongly for participants to ensure fair access to NDIS supports, housing, health services, and justice pathways. We help participants and families navigate complex systems with clarity and confidence.
